摘 要:目的:了解基质辅助激光解析离子-飞行时间质谱仪(MALDI-TOF-MS ,MS)在临床分离革兰阳性球菌鉴定中的应用,为临床医师正确诊断提供科学依据。方法回顾性分析2013年1月-2014年9月临床标本经 MS鉴定为革兰阳性球菌,将所得菌质谱峰图与数据库中峰图进行比较,分析得出鉴定结果。结果 MS 共鉴定出革兰阳性球菌3812株,葡萄球菌属2094株共18种,占54.93%,居前3位分别为表皮葡萄球菌、金黄色葡萄球菌、人葡萄球菌,分别占20.2%、14.7%、9.9%;肠球菌属1254株共10种,占32.90%;其中屎肠球菌占19.9%、粪肠球菌占11.8%;链球菌属374株共19种,占 9.81%,以无乳链球菌检出最多占3 .1%;其他革兰阳性球菌90株包括9个属占2.36%,使用 MS 后除了能鉴定出临床常见革兰阳性球菌,还增加了少见菌的鉴定种类。结论 MS鉴定革兰阳性球菌数量多、种类多,为临床治疗、鉴别感染病原菌提供了快速的筛选方法。
OBJECTIVE To investigate the performance of matrix-assisted laser desorption-ionization time-of-flight (MALDI-TOF) mass spectrometry in rapid identification of gram-positive cocci so as to provide scientific basis for correct clinical diagnosis .METHODS The gram-positive cocci that were identified from clinical specimens by the MALDI-TOF mass spectrometry were retrospectively analyzed .The mass spectrometer was placed for the identifi-cation after it was dried ;the acquired bacterial mass spectral peak figure was compared with the peak figure in the databases ,and the results of the identification were analyzed .RESULTS A total of 3 812 strains of gram-positive cocci have been identified with the use of MALDI-TOF mass spectrometry ,including 2 094 (54 .93% ) strains of Staphylococcus spp , with 18 species involved ; the Staphylococcus epidermidis , Staphylococcus aureus , and Staphylococcus hominis ranked the top three species ,accounting for 20 .2% ,14 .7% ,and 9 .9% ,respectively ;there were 1 254 (32 .90% ) strains of Enterococcus spp (10 species) ,among which the Enterococcus f aecium ac-counted for 19 .9% ,the Enterococcus f aecalis 11 .8% ;there were 374 (9 .81% ) strains of Streptococcus sp p (19 species) ,among which the Streptococcus agalactiae was the predominant species ,accounting for 3 .1% ;there were 90 (2 .36% ) strains of other gram-positive cocci ,including 9 genera .The MALDI-TOF mass spectrometry could not only identify the common clinical isolates of gram-positive cocci but also discriminate the species of rare bacteria .CONCLUSION The MALDI-TOF mass spectrometry can identify a variety of species of gram-positive co-ci ,and it may facilitate the clinical treatment an the rapid screening of the pathogens causing infections .
